Direct Primary Care

  • Primary Care members enjoy direct access to their care team. We provide the full spectrum of family medicine services detailed below. Our providers are collaborative and operate at the highest scope of their licenses. We strive to reduce the need for urgent care and emergency room visits. Whenever possible, we also avoid expensive specialty referrals and delayed care by taking the time to manage health concerns in our clinic.

    Primary Care members also have access to discounted medical services such as IV therapy, ozone, Botox, peptides and other unique regenerative medicine therapies.

Signature Membership

$250 per month
  • Signature members receive the entire primary care suite of services. After an initial investment of $499, signature members complete comprehensive lab evaluation, Evolt body composition scan and health coach consultation. Your provider specializing in hormone optimization, functional nutrition and regenerative medicine will follow-up for a consultation to review your lab report. Together, you will customize a care plan specific to your needs and health goals.

    Signature members also receive quarterly hormone optimization lab work, ongoing health coach access and regular body composition scans. Medically necessary labs to manage your customized care plan are also included.
